Cardiac Rehabilitation

The Community Hospital offers comprehensive Cardiac Rehabilitation for people with heart disease or those at high risk for developing it. These comprehensive programs combines cardiovascular conditioning exercise with education and counseling in order to make lifestyle changes necessary to reduce the risk of heart disease or slow its progression. Cardiac Rehabilitation is offered in three phases. Physician referral is required for admission into each phase. Cardiac Rehabilitation nurses and exercise physiologists help cardiac patients return to normal, active and productive lives. Phase 1 and 2 Cardiac Rehab programs are offered in the hospital, while Phase 3 is offered at the Lake Business Complex, 9200 Calumet Ave., Suite N100, located across from Community Hospital in Munster, IN. The Phase 3 program offers programming for people with heart disease, diabetes, peripheral artery disease, as well as individuals that are greater risk for these diseases due to high blood pressure, high cholesterol, overweight and other risk factors. Once a patient graduates from Phase 3, he or she has the opportunity to move into a less structured, but supervised adult fitness program at Fitness Pointe®.
