Cardiac Rehabilitation

The Community Hospital offers comprehensive Cardiac Rehabilitation for people with heart disease or those at high risk for developing it. The comprehensive program combines cardiovascular conditioning exercise with education and counseling in order to make lifestyle changes necessary to reduce the risk of heart disease or slow its progression. Cardiac rehabilitation is offered in three phases. Physician referral is required for admission into each phase. Cardiac rehabilitation nurses and exercise physiologists help cardiac patients return to normal, active and productive lives. Phase I and II Cardiac Rehab programs are offered in the hospital, while Phase III is offered at the Community Hospital Fitness Pointe®, 9950 Calumet Ave. in Munster, IN. Once a patient graduates from Phase III, he or she has the opportunity to move into a less structured, but monitored adult fitness program either as a general Fitness Pointe® member, or in our Rehab Plus program.
