Getting the best care for diabetes begins with the choices you make every day - what to eat, how to stay physically active, taking medications and checking blood sugar levels. Community Hospital offers a variety of educational, support and screening programs to help people with diabetes stay healthy and prevent long-term complications such as heart disease, stroke, or conditions of the eye, foot, and kidneys.
The Diabetes Center programs of Community Hospital focus on these seven self-care behaviors: healthy eating, being active, monitoring, taking medications, problem solving, healthy coping and reducing risks. We work together as a team to address each person's unique concerns, utilizing the expertise of: the patient's Physician; Certified Exercise Physiologist; Pharmacist; Registered Dietitian; Registered Nurse; and Certified Diabetes Educators.
