Diabetes Self-Management Education
Community Hospital Diabetes Self-Management Education has been Recognized by the American Diabetes Association for Quality Self-Management Education*. This three-part series addresses diabetes disease process, prevention and detection of complications, nutrition therapy, physical activity, medications and monitoring. Individual Diabetes Self-Management Education
Community Hospital Individual Diabetes Self-Management Education has been Recognized by the American Diabetes Association for Quality Self-Management Education*. We offer individual training for insulin administration, self blood glucose monitoring or for anyone who has special requirements. Most insurance plans cover this education program. Gestational Diabetes
Education Community Hospital Gestational Diabetes Education has been Recognized by the American Diabetes Association for Quality Self-Management Education*. Individual sessions with a diabetes education and registered dietitian to review: diabetes disease process and treatment, meal planning, blood glucose monitoring, exercise, stress and insulin therapy. Diabetes Management with Exercise
This four-week diabetes management program features exercise, advanced education and support. In a medically-supervised setting, learn and experience the benefits of exercise in controlling blood sugar. A physician referral is necessary and possible stress test and lab work may be required for participation. For more information or to enroll, call 219-934-2830.

Nutritional Counseling
To help achieve goals for weight loss, a normal blood sugar level and a healthier lifestyle, Community Hospital offers nutritional counseling on an individual or group basis. A Registered Dietitian and Certified Diabetes Educator helps participants develop a personalized meal plan matched to their lifestyle and medication plan. Screening Programs
The hospitals of the Community Healthcare System offer a variety of educational programs and screenings offered at low or no-cost to the public. Blood pressure checks, glucose and cholesterol testing, foot exams, heart and Peripheral Artery Disease (PAD) screenings are among the screenings offered regularly through the hospitals of the Community Healthcare System.
