Home Healthcare Servicesare available on an intermittent basis to patients at their home, a skilled or long term nursing facility or anywhere the patient considers their home. These services range from skilled nursing, to therapy, to personal care. Patients who receive home healthcare services are under the care of a physician who order the care we provide. Home healthcare services are provided by: Community Home Health Services, Home Health of St. Catherine Hospital and Home Health of St. Mary Medical Center.
Community Health Services (private duty) offers supportive care to individuals who need assistance with the tasks of everyday living to maintain their health and independence. These services can be provided in the home, in the hospital or a long term or nursing facility. Services range from attending to personal care needs, to housekeeping, transportation and/or companion care. Our support services are available 24-hours a day, 7 days a week, or on an intermittent basis. Community Health Services also offers nursing blood pressure screening, law draws, medication reconciliation and setting up medication boxes.
