Home Health Services
The Home Health Services of the Community Healthcare System bring to patients in their home the same high quality care they have come to expect from the Community Healthcare System. Our home health services are provided through the following licensed Medicare, Medicaid agencies: Community Home Health Services; Home Health of St. Catherine Hospital; and Home Health of St. Mary Medical Center.
Our health professionals work with physicians, patients and their families to provide comprehensive care that promotes healing and independence. Our home health services enable patients to receive long- or short-term professional medical care in the comfort and security of their own homes. Services such as skilled nursing, therapy and personal care are coordinated to meet the unique needs of patients and their families.
Home care services are available to patients at home or in the hospital 24-hours a day or on an intermittent basis. Our warm, caring professionals can help identify home health care needs; administer and manage complex treatments; help patients and families adjust to home health care; and educate patients and families about prescribed diets, medications and other aspects of the medical regimen.

- On-call nursing, 24 hours-a-day, seven days-a-week

- Home hyperalimentation/IV services

Community Companion Care Program
The Companion Care Program offers families the additional support they may need to help a loved one remain in the home during an illness or recuperation. For an hourly rate, home companions can come into the home to assist with a variety of tasks, including housekeeping, cooking and home management. Whether it's a new mom who needs extra help or a senior who would benefit from some fellowship, our caring companions help to put families at ease.
