Kudos to the Transitional Care Center at Community
Kudos to the Transitional Care Center at Community, which, for the second year in a row, has received a perfect survey by the Indiana State Department of Health. Three state surveyors visited the facility on February 1-2 and observed medication passes, assessed closed and open medical records and met with residents and their families.
