9/14/2013 - Coronary Health Appraisal
Determines your risk for heart disease and other related medical conditions. Screening includes: cholesterol (total, HDL, LDL, triglycerides), hemoglobin A1C and average estimated glucose, metabolic syndrome, blood pressure, body mass index, and a Heart Health Profile. 10-12 hour fasting is required.
The screening is held in the Outpatient Service area on the first floor at Community Hospital. Please use the Outpatient Admitting entrance that faces Columbia Avenue. It is labeled entrance "B". Immediately turn to the left and follow the carpeted hallway directly into the Outpatient Services area. There is an overhead sign above the entrance to that waiting area. It reads…Outpatient Services (Laboratory, EKG, X-ray).
