Pharmacy Residency Preceptors

NectariaNectaria Bonner, Pharm.D., MBA 
Nectaria is the Inpatient Pharmacy Manager at Community Hospital and has been practicing since 2002. She oversees the day-to-day pharmacist operations. She received her Bachelor’s degree from St. Louis College of Pharmacy, followed by her Doctor of Pharmacy degree from the University of Florida, and her MBA in health care administration from Lewis University. 

Sean ChantarapanontSean Chantarapanont, Pharm.D., BCPS 
Sean serves as the PGY1 Residency Program Director and is the Clinical Pharmacy Manager at Community Hospital. He is a board certified pharmacotherapy specialist and has interests in Pharmacy Administration, Medication Safety and Oncology. He received his Doctor of Pharmacy degree from the University of Illinois at Chicago, and completed his PGY-1 and PGY-2 Health System Pharmacy Administration Residencies at Northshore University HealthSystem in Evanston, IL. 

Elizabeth ClementsElizabeth Clements, Pharm.D. 
Beth has been the director of pharmacy for Community Hospital since 2009. She previously worked as clinical manager at Spectrum Health, and emergency medicine specialist at Spectrum Health and Detroit Medical Center. She received her Doctor of Pharmacy degree from University of Illinois at Chicago and Bachelor of Science in Biology at University of Illinois at Champaign-Urbana. She completed a PGY1 at Meriter Hospital in Madison, WI.

MikeMichael DiNatale, Pharm.D. 
Mike is a clinical pharmacist who has been practicing at Community Hospital since 1992. His main focuses include critical care, infectious disease, nutritional support, anticoagulation and pharmacokinetics. He participates in the Infection Control, Medication Safety, Sedation and Pharmacy & Therapeutics committees. He completed his pharmacy education at Purdue University in 1992.

TimTim Gardiner, Pharm.D., BCPS 
Tim is the Medication Safety Officer at Community Hospital. He has been practicing at Community Hospital since 1998 and is a board-certified pharmacotherapy specialist focusing in cardiovascular care, investigational medication management, nutritional support, and neonatal and pediatric care. He participates in the Critical Care Committee, Code Blue Team, Investigational Review Board, and Pharmacy & Therapeutics committee. He completed his pharmacy education at Ferris State University in 1997, followed by his PGY-1 Pharmacy Practice Residency at Franciscan St. Margaret Health in 1998.  

caitlin jacobCaitlin Jacob, Pharm.D.
Caitlin is a Clinical Pharmacist specializing in Cardiology at Community Hospital. She earned her Doctor of Pharmacy degree from the University of Cincinnati James L. Winkle College of Pharmacy in 2016. Following graduation, she completed her PGY-1 and PGY-2 specializing in Critical Care at the University of Cincinnati Medical Center.

Neal LyonsNeal Lyons, Pharm.D., BCPS
Neal is the Clinical Pharmacist specializing in Emergency Medicine at Community Hospital. He graduated pharmacy school from the University of Illinois at Chicago in 2011 and completed his PGY-1 at the University of New Mexico Hospital. He completed his PGY-2 in Emergency Medicine at Advocate Christ Medical Center and stayed on as the Emergency Medicine Clinical Coordinator for 4 years. His professional interests include: resuscitative science, toxicology, pediatrics, and pharmacokinetics.

BhavikBhavik Nana, Pharm.D., BCPS
Bhavik graduated from Purdue University School of Pharmacy in 2010. He completed a one year post-graduate residency at Franciscan St. Margaret Health in 2011. Bhavik is a board-certified pharmacotherapy specialist and currently practices as a General Medicine Clinical Pharmacy Specialist. His clinical interests include anticoagulation, pain management and ambulatory care.

Zach RosenfeldtZach Rosenfeldt, Pharm.D., BCPS
Zach graduated from the Rockford campus University of Illinois Chicago College of Pharmacy in 2016. Following graduation, he completed a one year post-graduate residency at Memorial Medical Center in Springfield Illinois, affiliated with Southern Illinois University. He joined the CHS pharmacy team in July of 2017 and has interest areas in critical care and emergency medicine.

AlexAlex Szewciw, Pharm.D. 
Alex is a staff pharmacist who currently practices in the Ambulatory Anticoagulation Clinic at Community Hospital. A graduate of Purdue University, he earned his Doctor of Pharmacy degree in 2010 and has been involved in multiple anticoagulation clinics since 2012. In addition to his experiences at Community Hospital, Alex has ample retail, Medication Therapy Management, clinical, and ambulatory experience. 

MattJunyu (Matt) Zhang, Pharm.D., BCPS
Matt graduated from the University of Illinois at Chicago College of Pharmacy in May 2015. He finished his post graduate residency at Community Hospital in July 2016. His clinical interests include ambulatory care, oncology, and critical care. Matt currently practices as Clinical Pharmacist with a specialty focus in Oncology at Community Hospital.

Mary graduated from Purdue University College of Pharmacy with her PharmD in May 2018. Her interests include pharmacy administration, medication safety and critical care. In addition to pharmacy, she also enjoys reading, cycling and spending time with family and friends.

aaron hillAaron Hill, Pharm.D.
Aaron graduated from Purdue University College of Pharmacy in May 2018. His areas of interest include infectious disease, critical care and ambulatory care. Aaron has previous retail pharmacy experience and enjoys spending time outdoors fishing, hiking and kayaking. Aaron hopes to pursue a position as a clinical pharmacist after completion of his PGY-1 residency.

rebecca hiltonRebecca Hilton, Pharm.D.
Rebecca graduated from Butler University College of Pharmacy in May 2018. Rebecca hopes to complete a PGY-2 specialty residency in Critical Care or Internal Medicine. In her free time, she enjoys attending concerts, watching Grey's Anatomy and spending quality time with her co-residents.

allyson trevinoAllyson Trevino, Pharm.D.
Allyson graduated from Purdue University College of Pharmacy in May 2018. Her interests include general medicine, critical care and ambulatory care. Allyson hopes to pursue a career as a clinical pharmacist after completion of her PGY-1 residency. In addition to spending time with her co-residents, she enjoys hiking the dunes and watching cheesy sci-fi movies.
