Pharmacy Residency Preceptors

NectariaNectaria Bonner, Pharm.D., MBA 
Nectaria is the Inpatient Pharmacy Manager at Community Hospital and has been practicing since 2002. She oversees the day-to-day pharmacist operations. She received her Bachelor’s degree from St. Louis College of Pharmacy, followed by her Doctor of Pharmacy degree from the University of Florida, and her MBA in health care administration from Lewis University. 

SeanSean Chantarapanont, Pharm.D., BCPS 
Sean serves as the PGY1 Residency Program Director and is the Clinical Pharmacy Manager at Community Hospital. He is a board certified pharmacotherapy specialist and has interests in Pharmacy Administration, Medication Safety and Oncology. He received his Doctor of Pharmacy degree from the University of Illinois at Chicago, and completed his PGY-1 and PGY-2 Health System Pharmacy Administration Residencies at Northshore University HealthSystem in Evanston, IL. 

ElizabethElizabeth Clements, Pharm.D. 
Beth has been the director of pharmacy for Community Hospital since 2009. She previously worked as clinical manager at Spectrum Health, and emergency medicine specialist at Spectrum Health and Detroit Medical Center. She received her Doctor of Pharmacy degree from University of Illinois at Chicago and Bachelor of Science in Biology at University of Illinois at Champaign-Urbana. She completed a PGY1 at Meriter Hospital in Madison, WI.

MikeMichael DiNatale, Pharm.D. 
Mike is a clinical pharmacist who has been practicing at Community Hospital since 1992. His main focuses include critical care, infectious disease, nutritional support, anticoagulation and pharmacokinetics. He participates in the Infection Control, Medication Safety, Sedation and Pharmacy & Therapeutics committees. He completed his pharmacy education at Purdue University in 1992.

Stephanie DwyerStephanie Dwyer, Pharm.D.
Stephanie is a Clinical Specialist Pharmacist and practices in the Cardiovascular Care Units at Community Hospital. She received her Pharm.D. degree from the University of Illinois at Chicago College of Pharmacy. She completed her PGY1 training at the Captain James A. Lovell Federal Health Care Center and her PGY2 in Cardiology Pharmacy at the University of Illinois at Chicago. Her clinical areas of interest include heart failure, antiarrhythmic medications, and acute coronary syndromes.

TimTim Gardiner, Pharm.D., BCPS 
Tim is the Medication Safety Officer at Community Hospital. He has been practicing at Community Hospital since 1998 and is a board-certified pharmacotherapy specialist focusing in cardiovascular care, investigational medication management, nutritional support, and neonatal and pediatric care. He participates in the Critical Care Committee, Code Blue Team, Investigational Review Board, and Pharmacy & Therapeutics committee. He completed his pharmacy education at Ferris State University in 1997, followed by his PGY-1 Pharmacy Practice Residency at Franciscan St. Margaret Health in 1998.  

Neal LyonsNeal Lyons, Pharm.D., BCPS
Neal is the Clinical Pharmacist specializing in Emergency Medicine at Community Hospital. He graduated pharmacy school from the University of Illinois at Chicago in 2011 and completed his PGY-1 at the University of New Mexico Hospital. He completed his PGY-2 in Emergency Medicine at Advocate Christ Medical Center and stayed on as the Emergency Medicine Clinical Coordinator for 4 years. His professional interests include: resuscitative science, toxicology, pediatrics, and pharmacokinetics.

BhavikBhavik Nana, Pharm.D., BCPS
Bhavik is responsible for providing clinical pharmacy services at Community Hospital, including pharmacokinetic, TPN, and anticoagulation dosing and management; drug information and educational services; and participation on the Pharmacy & Therapeutics committee. He completed his pharmacy education at Purdue University in 2010, followed by his PGY-1 Pharmacy Practice Residency at Franciscan St. Margaret Health in 2011. He also practices at the Ambulatory Anticoagulation Clinic at Community Hospital.

Zach RosenfeldtZach Rosenfeldt, Pharm.D.
Zach graduated from the Rockford campus University of Illinois Chicago College of Pharmacy in 2016. Following graduation, he completed a one year post-graduate residency at Memorial Medical Center in Springfield Illinois, affiliated with Southern Illinois University. He joined the CHS pharmacy team in July of 2017 and has interest areas in critical care and emergency medicine.

AlexAlex Szewciw, Pharm.D. 
Alex is a staff pharmacist who currently practices in the Ambulatory Anticoagulation Clinic at Community Hospital. A graduate of Purdue University, he earned his Doctor of Pharmacy degree in 2010 and has been involved in multiple anticoagulation clinics since 2012. In addition to his experiences at Community Hospital, Alex has ample retail, Medication Therapy Management, clinical, and ambulatory experience. 

MattJunyu (Matt) Zhang, Pharm.D., BCPS
Matt graduated from the University of Illinois at Chicago College of Pharmacy in May 2015. He finished his post graduate residency at Community Hospital in July 2016. His Clinical interests include ambulatory care, oncology, and critical care. Matt currently practices as Clinical Pharmacist with a specialty focus in Oncology at Community Hospital.

Nicole JonesNicole Jones Latimore, Pharm.D.
Nicole graduated from Chicago State University College of Pharmacy in May 2017. She has previous hospital and retail pharmacy experience and enjoys amusement parks and picnics. Nicole hopes to complete a PGY-2 specialty residency and become a clinical pharmacist. Her areas of interest include Critical Care, Emergency Medicine, and Cardiology.

Jessica MorrisonJessica Morrison, Pharm.D.
Jessica graduated from University of Findlay College of Pharmacy in May 2017. Her interests include general medicine, pediatrics, cardiology, and oncology. She hopes to pursue a PGY-2 in a specialty area of her interest.

 

Stephanie PitmanStephanie Pitman, Pharm.D.
Stephanie graduated from Purdue University College of Pharmacy in May 2017. Her areas of interest include oncology and infectious disease. She hopes to pursue a career as a clinical pharmacist after completion of her PGY-1 residency.
