Cardiac and Pulmonary Rehabilitation

The hospitals of Community Healthcare System offer an aggressive disease management Cardiac and Pulmonary Rehabilitation for people with heart, vascular and lung disease or those at high risk for developing it.

Cardiac Rehabilitation programs combine cardiovascular conditioning exercise with education and counseling in order to make lifestyle changes necessary to reduce the risk of heart disease or slow its progression. Pulmonary Rehabilitation offers support and motivation for building healthier lives for patients with pulmonary disorders, or those who are recovering from other cardiopulmonary conditions.

Individuals benefit from experienced cardiac nurses, exercise physiologists, respiratory therapists and physicians who customize a program of exercise, education and counseling to help individuals reduce the risk of disease or slow its progression.

Your health care team will evaluate your health to make sure you are ready to participate in Cardiac and Pulmonary Rehabilitation. Various phases are offered.

Phase 1
During your hospital stay, a cardiac rehab specialist will direct your physical activities and education. You will receive information about the recovery process and learn how to reduce your future risk of cardiovascular disease.

Phase 2
After your hospital discharge, you can enroll in the Phase 2 program. Once the doctor’s order is received, you will be scheduled for an initial evaluation and orientation with a cardiac rehab specialist. Medicare and most insurance carriers provide coverage for Phase 2. The program meets three times per week.

Phase 3
This program is designed to help individuals acquire the skills and knowledge needed to work towards heart disease management and prevention. Our program combines cardiovascular and muscular strength conditioning along with education and individual risk factor counseling, all within a supportive supervised environment with regular monitoring of heart rate, heart rhythm and blood pressure. Oxygen saturation and blood sugar can be measured as needed.

Referring physicians are provided with regular progress reports regarding the status of patients enrolled in Cardiac Rehabilitation. These reports detail the patient's plan of care, patient goals and level of progress.
Peripheral Artery Disease Rehabilitation Program (PAD)
Screenings and supervised exercise therapy under a physician direction are offered to patients with PAD.

How to Enroll

If you have heart or lung disease, had a heart procedure or are at risk, Cardiac and Pulmonary Rehabilitation offers support and motivation for building a healthier life. To enroll, ask your doctor. A physician referral is needed.
