Heart Failure Treatment Program

Heart Failure is a condition in which the heart has weakened and has difficulty pumping blood throughout the body. The Heart Failure Program offers support, treatment and education to improve the quality of life for heart failure patients. The program is designed to support your doctor's treatment plan and provide the tools and assistance needed to stay healthy and out of the hospital.


Education is provided to hospitalized heart failure patients and their families. To address the various needs of our heart failure patients, we offer patients support and services from specially trained nurses, clinical dieticians, pharmacists, social workers and physician oversight.


Upon physician referral, the treatment center staff focuses on your medicine, diet, exercise and lifestyle changes to improve your heart health. Follow-up may include continued visits to the center, or a home telephone monitoring system.

The staff along with your physician will evaluate and monitor your condition through:

  • Physical Exam
  • Laboratory Tests
  • Intravenous (IV) Medications to help lessen symptoms
  • Oral Medication Therapy
  • Impedance Cardiography Test (ICG), a noninvasive test to provide information about your heart's ability to deliver blood to the rest of your body.


A home telephone monitoring system of your weight and symptoms is available. This program enables our staff to monitor your progress and better predict how you are responding to treatment or quickly alert the physician if problems are developing. The program empowers the patient to achieve their individual goals and understand their condition through appropriate education, nutritional counseling, and individual health assessment.

Patient Family Education and Support

The heart failure program staff focuses on medication, diet, exercise, positive lifestyle changes and symptom identification and management to improve the patient's heart health. Support group featuring guest speakers on topics of specific interest to heart failure patients are offered to help patients and families better understand the disease process.
