Palliative Care is a free service offered by the hospitals of Community Healthcare System, dedicated to improving the quality of life for patients and their families facing the stress of a life-limiting illness. The program provides an extra layer of information and support, both in the hospital and in the community setting, in coordination with your current course of treatment. Palliative Care works to enable patients to live their lives as fully and comfortably as possible, treating each person as a whole, not just their illness.

A Multidisciplinary Team

Each team member offers an important and unique set of skills, working together with your physicians to ensure the highest quality care. Team members include:

  • Your physician treatment team
  • Palliative care nurse practitioner
  • Registered nurses
  • Social worker
  • Physical, occupational and speech therapists
  • Respiratory therapists
  • Dietitians
  • Chaplains
