Our Approach

The Heart Failure Program offers support, treatment and education to improve the quality of life for heart failure patients.
Our program is designed to support your doctor's treatment plan and provide the tools and assistance needed to stay healthy and out of the hospital.

Heart Failure is a condition in which the heart has weakened and has difficulty pumping blood throughout the body. Our program supports your doctor's treatment plan and provides our patients with the tools and assistance to stay healthy and out of the hospital.

You are a greater risk for heart failure, if you have had any of the following:

Treatments

IMPLANTABLE DEVICE

Placed in the body to keep heart rate at normal beat and rhythm, such as a pacemaker, defibrillator and CardioMEMS™ - the first FDA-approved heart failure monitoring system proven to significantly reduce hospital admissions.

INTRAVENOUS MEDICATIONS

Injection of a medication directly into the veins.

MEDICATION MANAGEMENT

Anticoagulation therapy, education and counseling.

ORAL MEDICATION THERAPY

Pharmaceutical treatment to slow disease progression.

Inpatient

To address the various needs of our heart failure patients, we offer patients education, support and services from specially trained nurses, clinical dietitians, pharmacists and social workers with physician guidance.

Outpatient

Upon physician referral, the treatment center staff focuses on medicine, diet, exercise and lifestyle changes to improve your heart health. Follow up may include continued visits to the center or a home telephone monitoring system.

Telemanagement

A home telephone monitoring system of your weight and symptoms is available. This program enables our staff to monitor your progress and better predict how you are responding to treatment or quickly alert the physician if issues develop. The program empowers our patients to achieve individual goals and understand their condition through education, nutritional counseling and individual health assessment.

Patient Family Education and Support

Heart Failure Program staff focus on medication, diet, exercise, positive lifestyle changes and symptom identification and management to improve the patient's heart health. A support group is offered to heart failure patients and their families to help them better understand the disease process.
